From January 2 to March 16, 2020

Objective

1. To promote their good self-care behavior and blood pressure control (BP <140/90 mmHg) among older adults with hypertension.

2. To promote their good dietary behavior and glycemic control (HbA1C < 7%) among older adults with type2DM

Target group

1. Thirty older adults with hypertension were selected based on the inclusion criteria. The inclusion criteria were aged between 60-79 years, determined as having hypertension (BP ≥ 140/90 mmHg) and taking anti-hypertensive medications, determined as with or without severe comorbidities, determined as cognitive intact, able to use a telephone, and living in three communities.

2. Thirty older adults with type2DM were selected based on the inclusion criteria. The inclusion criteria were aged between 60-79 years, determined as having uncontrolled DM (Glucose ≥ 130 mg%), determined as cognitive intact, able to use a telephone, and living in three communities.

Activities

Activity 1: Self-management for older adults with hypertension in the community

1. We had a meeting with the Health Volunteers in order to inform about the objectives, sample recruitment, and setting (Feb 3, 2020)

2. We conducted the activities for older adults with hypertension in the community as followed;

  • 1st week (Feb 16, 2020): Education and self-management skill training related to diet, exercise, stress management, taking medication, and blood pressure measurement for older adults with hypertension. Then, we asked them to set goals and their action plans.
  • 4th week (Mar 15, 2020): Evaluation their self-care behaviors and blood pressure control among older adults with hypertension at 4th week. Then, we performed a focus group interview.
  • 8th week (Apr 19, 2020): Evaluation their self-care behaviors and blood pressure control among older adults with hypertension at 8th week.

3. We used telephone follow-ups in order to ask their self-evaluation based on the action plan as well as providing support (Mar1, 2020; Mar 8, 2020)

Activity 2: Self-management for older adults with type2DM in the community

1. We had a meeting with a member team in order to inform their role and responsibilities (Feb 13, 2020)

2. We conducted the activities for older adults with hypertension in the community as followed;

  • 1st week (Feb 22, 2020): Education and self-management skill training related to diet for older adults with type2DM. Then, we asked them to set goals and their action plans.
  • 12th week (Mar 16, 2020): Evaluation their dietary behaviors and glycemic control among older adults with type2DM at 12th week.

3. We used telephone follow-ups in order to ask their self-evaluation based on the action plan as well as providing support (Feb 29, 2020; Mar 7, 2020; Mar 14, 2020; Mar 21, 2020)

Output/Outcome

1. There was 84 percent of older adults with hypertension having good self-care behavior.

2. There was 80 percent of older adults with hypertension having blood pressure control.

3. There was 80.76 of older adults with hypertension having BP < 135/85 mmHg.

4. There was 65.22 of older adults with type2DM having good dietary behavior.

5. There was 33 of older adults with type2DM having blood glucose < 130 mg%

6. There was 54 of older adults with type2DM having HbA1C < 7.5%
